Brief notes from each meeting we have.

To arrange home visits.
To record clinical events such as goals, progress and achievements.

I may share this information with mental health or social support services, but will always discuss this with you first.

How I store your personal information:

Your information is stored securely in digital format.

I will keep your personal information for 5 years. I will then dispose of your information by deleting all copies, including anywhere it is backed up.
